NEW (1st time) AUTO INJURY MEDICAL MASSAGE please email alicia@musclmendmassage.com to request an appointment. Prior to booking, the following is required:

-Massage Rx from your Primary Care Physician or Chiropractor

-Auto insurance company name

-Incident claim number

-Claim Adjuster’s name, phone number and email

• Please book online and manage your appointments through WWW.MASSAGEBOOK.COM
• Cancel/reschedule online 24hrs or more in advance without penalty at WWW.MASSAGEBOOK.COM
• Less than 24hrs notification to cancel/reschedule is subject to a late cancel fee equal to the cost of service booked
• If No Show/Late Cancel fee is not paid within 24hrs, you could be flagged from booking online in the future
• Payments are accepted through Square via Debit, Credit, HSA/FSA cards
• Pre-paid Services and Gift Certificate sales are Final and Non-Refundable
• Online intake form must be completed prior to first appointment or updated upon request
• Please arrive at least 5 min prior to your scheduled appointment time
• If you are late to your appointment, your time may be cut short in order to properly sanitize the treatment room for the next appointment
• If you have been exposed to COVID-19 within 5-7 days of your next scheduled appointment, please cancel your appointment ASAP and reschedule for a later date
