Prior to booking, the following is required:

• Massage Rx from your Primary Care Physician or Chiropractor
• Auto insurance company name
• Incident claim number
• Claim Adjuster’s name, phone number and email

Book online and create a massagebook.com profile to manage your appointments through www.massagebook.com

Cancel/reschedule online 24hrs or more in advance without penalty at www.massagebook.com

Less than 24hrs notification to cancel/reschedule is subject to a late cancel fee equal to the cost of service booked

If No Show/Late Cancel fee is not paid within 24hrs, you could be flagged from booking online in the future

If you are booking an appointment for a friend or family member on your www.massagebook.com profile, please notify your therapist in advance via text or email

Payments are accepted through Square via Debit, Credit, HSA/FSA cards. Cash or check is accepted in person

Pre-paid Services and Gift Certificate sales are Final and Non-Refundable

Online intake form must be completed prior to first appointment or updated upon request

Please arrive at least 5 min prior to your scheduled appointment time

If you are late to your appointment, your time may be cut short in order to properly sanitize the treatment room for the next appointment
